CHAPTER QUILT SHOW  or EDUCATION EVENT AWARD MONEY REQUEST FORM


Each Chapter of the Utah Quilt Guild may apply for award money for a Quilt Show or education event that the chapter is sponsoring.  The $50 grant is allowed one (1) time each year. Award Money must be used for awards and/or prizes or for an education event. Checks not cashed within 90 days after issue date, will not be valid

Application should be submitted to Utah Quilt Guild at least two (2) months before proposed show.  This will allow the UGQ Board time to approve and process the application. 

Group must be a Chartered Chapter of UQG.

Do not use this from for Area Show  money request. Only the UQG board member over the area where the show is located in can submit for Area Show funds.
